Two-clinician charting with a scribe. Probing at six sites per tooth, bitewings at 18-month intervals, bleeding on probing recorded separately from pocket depth.

Sequencing logic
Phase I resolves active caries and infection. Phase II rebuilds function and aesthetics. Nothing definitive is placed until the periodontium re-evaluates stable at eight weeks.
Materials
Posterior restorations in bulk-fill composite; lithium disilicate for single crowns. Metal-free unless bruxism history dictates otherwise.
